Once a service commences
• Co-ordinating office staff will keep in regular contact with the staff and the service user in order to identify and eliminate any issues
• Be visited by a member of the care staff team 3 monthly to review the care plan and risk assessment; discuss how the service is going; update any other relevant paperwork
If there is a to be a change to the service the service user due to sickness or unforeseen circumstances we will;
* Call the service user at the earliest opportunity and all others this may impact on
* Inform them of the problem and how soon they can expect a service
* Arrange for support to go into them at the earliest opportunity
* Keep the service user/ informed at all times
* Give 7 days notice for cessation of the service/ or any changes to the times of support given
